Renal failure in atherosclerotic renovascular disease: pathogenesis, diagnosis, and intervention.
نویسنده
چکیده
Atherosclerotic renovascular disease (ARVD) is increasingly recognised as an important cause of both chronic and end stage renal failure. These patients tend to do badly on dialysis, which reflects their systemic atherosclerotic burden. In an eVort to delay and perhaps prevent their need for renal replacement therapy, some patients are subjected to a variety of medical, radiological and surgical interventions, although evidence for each is sparse. The purpose of this review is to describe the epidemiology and pathophysiology of renal failure in ARVD, discuss the available diagnostic techniques, consider the evidence for benefit from intervention in the context of pathogenesis and finally, identify those gaps in our knowledge which impede the practice of evidence based medicine.

Atherosclerotic renal artery stenosis and renovascular hypertension: clinical diagnosis and indications for revascularization.
Atherosclerotic renal artery stenosis (RAS) is relatively common and often associated with reversible hypertension, progressive renal insufficiency, and/or coronary-independent pulmonary edema. Not all RAS is associated with renovascular hypertension. Historical and physical findings may suggest renovascular hypertension and warrant investigation for RAS. Noninvasive diagnostic imaging options ...
متن کاملAtherosclerotic renal artery stenosis: from diagnosis to treatment.
Renovascular hypertension represents a form of correctable hypertension and preventable renal failure. Such patients need to be identified early so that specific therapy can be instigated. Patient identification requires a high index of suspicion in patients with certain clinical features. Subsequent non-invasive imaging may result in angiography which is required for diagnostic purposes and fo...
متن کاملIschemic nephropathy: where are we now?
Identification and reversing the loss of kidney function beyond occlusive disease of the renal arteries poses a major clinical challenge. Recent studies indicate that atherosclerotic renal artery stenosis develops as a function of age and is commonly associated with other microvascular disease, including nephrosclerosis and diabetic nephropathy. The risks of renal artery stenosis are related bo...
متن کاملPrevalence of atherosclerotic renal artery stenosis in patients starting dialysis.
BACKGROUND Atherosclerotic renal artery stenosis (ARAS) can lead to end-stage renal failure (ESRF). We determined the prevalence of ARAS in patients 45 years of age or older starting renal replacement therapy. METHODS Forty-nine of 80 consecutive patients (37 males, 12 females) starting renal replacement therapy in our centre gave informed consent and underwent spiral computed tomographic ang...
متن کاملAtherosclerotic Renovascular Hypertension : Lessons from Recent Clinical Studies
Atherosclerotic renovascular hypertension is a form of secondary hypertension due to renal artery stenosis. After the introduction of medical therapy such as with statins and angiotensin blocking agents, it has been considered a very slowly progressive disease. In the 1990s, surgical methods were compared to radiological intervention and showed no additional benefits. Recent clinical data also ...
